Qualifications -Must be able to determine appropriate P.T., P.O.C. & D/C and make adjustments as necessary. -Must be able to accurately complete billing and timesheets. -BS degree in Physical Therapy and graduation from an accredited physical therapy program. -Experience in geriatric care is preferred. Must be able to document clearly to meet reimbursement requirements. Physical Therapy license from the State. Responsibilities 1. Performs an initial evaluation to assess the patients level of function. 2. Evaluates and develops a physical therapy patient care plan assigned by the physician. 3. Observes, records, and reports to the physician, nursing and rehabilitation team and other elated staff, as appropriate, the patients reaction to treatment and any changes in the patients condition. 4. Prepares and maintains accurate and timely patient documentation. This is a licensed professional who provides direct and indirect patient care and restoration of normal body functions after illness or injury. The individual performing this job routinely serves a geriatric population.
