CASE MANAGER
                                      ACCESS, INC.
                          POSITION: CASE MANAGER
The position of case manager provides casework management and supportive services for the clients of the emergency shelter. This is a full time hourly position requiring non-traditional hours and reports directly to the Assistant Director.
QUALIFICATIONS:
Required: Demands a mature, responsible individual with common sense. Demonstrated group management skills. Demonstrated crisis management skills. Excellent listening and communication skills, including skills in interviewing clients and case documentation. Builds nurturing relationships with clients for the purpose of identifying barriers to self-sufficiency, establishing goals for independence and coordinating support services. Assist other program staff in identifying and coordinating client support services. Requires a minimum of two years work experience in social service field. Excellent verbal and written communication skills. Works well in a team. High school diploma or equivalent. Demands a mature, responsible individual with common sense. Preferred: Bachelor’s Degree in Social Sciences. Demonstrated knowledge of community resources and an understanding of issues affecting homeless individuals. DUTIES AND RESPONSIBILITIES:
1. Meet new clients in the Emergency Shelter within 72 hours to develop a case management and goal
plan.
2. Conduct weekly individual case management sessions which includes ensuring client compliance with
their goals.
3. Maintain complete and accurate client case files.
4. Create a housing plan with client within two weeks of the client’s intake.
5. Create a personal plan with client within 3 weeks of the client’s intake.
6. Serve as a facilitator for client services.
7. Identify client needs, facilitate client support services and interventions.
8. Maintain a knowledge of community resources and networking contacts.
9. Attend community networking meetings as assigned by a supervisor
10. Carry out casework duties as detailed in Attachment A.
11. Attend Staff meetings as assigned.
12. Attend weekly supervision meetings.
Follow all agency policies and procedures.
Carry out all other duties as assigned by a supervisor.
Note: This position requires the employee to twist, bend, lift up to 50 pounds and to negotiate stairs.
ACCOUNTABILITY: The Case Manager position is directly accountable to the Program Supervisor.

CASEWORK RESPONSIBILITIES

Assess Families:
-Establish trusting relationships with families.
-Use agency assessment tools. (Intake forms)
-Objectively identify families’ stages of development.
-Identifies families’ strengths and weaknesses.
-Maintain documentation of case notes and one-on-ones.
-Summarize assessment findings in writing.
Plan with Families:
-Assist in identifying realistic goals for families.
-Assist in identifying action steps and strategies needed to achieve goals.
-Assist in establishing time table for goals.
-Assist in identifying the impact of goals fulfillment on family.
-Summarize plan in writing.
-Maintain complete and accurate documentation of goals and outcomes.
Provide Support to Families:
-Maintain weekly contact with all families.
-Assist families in monitoring progress toward goals.
-Assist in adjusting and modifying goals as needed.
-Maintain documentation of support contacts.
Resource Utilization:
-Establish positive partnerships with area resource providers.
-Understand eligibility requirements and screening procedures of area resources.
-Use agency and interagency referral tools.
-Systematically refer families to resources appropriate for families’ needs and levels of
development.
-Monitor families’ use of resources.
-Note all referrals in case files.
-Identify barriers to resource utilization and strategies to overcome barriers.
-Maintain regular contact with any other agencies working with families.
-Maintain regular contact with other ACCESS staff resources working with families.
