(NAC) is a health care and social service agency in Midtown Manhattan that serves medically fragile/physically disabled children and their families. NAC is currently seeking applicants for a RN – Pediatric Nurse/Clinical Nurse Specialist in its Medical Department.

Title:Â RN - Pediatric Nurse/Clinical Nurse Specialist
Â
Work Schedule: Full-time, Monday – Friday, 9:00 a.m. – 5:00 p.m.  Prefer ability to work every 6th or 7th Saturday and 11:00 a.m. – 7:00 p.m. once a week.

Supervisor:Â Director of Nursing and Clinic Administration
Â
Duties:
 Assess the medical needs of each child by reviewing their diagnosis, birth history, and inpatient and outpatient records. Initiate, maintain, and update health care plans for each client. Train foster and prevention families to care for their child in a home setting with reinforcement as needed. Make appropriate referrals for community-based care and provide follow-up as needed. Make periodic nurse home visits to children in the Prevention and Foster Care Programs; make nurse home visits at least monthly to foster care children with special medical needs. Fill out medical forms for school, day camps and treatment centers, medical records for courts for subsidy and adoption cases. Collaborate with social workers and doctors to assess child’s health care needs and problems, and to implement health care plans. Provide direct care to patients consisting of asthma treatments, catheterization, physical assessment for child abuse cases, vital signs, and any emergency care. Provide telephone triage and advice to biological and foster parents. Maintain records and provide follow-up on all children taking psychotropic medications and liaison to child psychiatrist. Provide pre-counseling to biological parents of children who are at risk for HIV and need to be tested; provide sexuality counseling to adolescents in prevention and foster care. Accompany NAC staff on field trips to monitor the medical needs of a child. This may require some weekend work.

Minimum requirements:
 NYS RN license   Must have 2 years post graduate experience in pediatrics and/or maternal child health field. Ability to be on call for medical emergencies after 9:00 a.m. – 5:00 p.m.Â
