Nurse Practitioner - Schermerhorn House
The Center for Urban Community Services (CUCS) is a national leader in the development of effective housing and service initiatives. Schermerhorn House, located in Boerum Hill, Brooklyn is a new supportive housing residence that will house 217 tenants many of whom have histories of mental illness, homelessness, substance abuse and/or HIV/AIDS. It will be the first CUCS project to open in that borough and is scheduled to open in October 2008.
Responsibilities will include providing primary care medical services in collaboration with physician to tenants residing at Schermerhorn House. On-site social service teams provide case management that support medical recommendations and treatment.
Requirements: Master's Degree and/or FNP or ANP. Five (5) years primary clinical care experience; experience working with homeless or formerly homeless and MICA clients preferred; bilingual Spanish/English preferred and computer literacy.
Schedule: Note: Flexible for a total of five (5) hours between 9:00am - 9:00pm (Monday - Friday) or 10:00am - 4:00pm (Saturday) Competitive salary and benefits. CUCS is committed to workforce diversity. EEO.
