LMSW Social Worker - Adult Outpatient
Under the supervision of the Program Director and in coordination with other program staff, provides and arranges social work services for program participants, a caseload of 28 clients. Hours for this positions are flexible, but prefer 11:30am to 7:30pm.
SPECIFIC DUTIES AND RESPONSIBILITIES:
· Assists clinical staff in the development of treatment plans, ensuring that such plans address all facets of a client's treatment -- mental health, substance abuse and education.
· Meets individually with new program participants during assessment phase.
· Conducts workshops and individual, group and family therapy sessions, as needed, in conjunction with other clinical staff.
· Works with treatment team in developing individual/family treatment plan and helps maintain positive working relationship with families.
· Works with treatment team to ensure that clients receive information/referrals regarding appropriate post-treatment services during the aftercare phase of the program.
· Responsible for overseeing and coordinating overall client case management to include interface with applicable state and local social services agencies, school administrators and teachers, and community based providers of services, as needed, for clients and their families. Such case management includes monitoring client progress in the program; academic, emotional and social progress during school day; and any client, family or outside problems or situations that may arise requiring intervention, referral to other agencies or reporting to social services personnel.
· Reviews clinical progress notes with other supervisory staff, and participates in the clinical case review process.
QUALIFICATIONS/REQUIREMENTS:
·MUST have prior knowledge of 822 Outpatient Programs.
· Must be an LMSW.
· Experience in drug rehabilitation, and experience working with dually-diagnosed clients.
· Must have excellent written and verbal communication skills and be able to work and communicate effectively with clients, families and school, agency and community representatives.
