JOB OPENING
TITLE: HOUSING SERVICES COORDINATOR – SENIOR HOUSING ASSISTANCE REPAIR PROGRAM
DEFINITION: Under the Direction of the Assistant Director of Housing, the Housing Social Worker provides assessment and limited care management to persons who are applying for and/or receiving services through the SHARP program.
QUALIFICATIONS:
Education:
● Bachelors’ Degree in social work or the related social sciences
● Three years experience in social services.
Experience:
● Preference will be given to persons with direct experience working with the elderly, people with disabilities and/or in housing related programs Personal Characteristics:
● High energy level
● Ability to respond to multiple requests for information and assistance
● Good organizational and people skills
Additional Requirements:
● Must have a valid driver’s license, a good driving record and continuous access to a fully insured vehicle.
DUTIES AND FUNCTIONS:
a. Reviews, contacts and prioritizes all referrals as assigned. Returns all phone calls within 24 hours.
b. Enters appropriate information in the data management systems.
c. Arranges visits to see the highest priority applicants first.
d. Visits applicants’ homes to complete assessment forms and supporting documentation, inspects the entire property to insure appropriateness for the program and assess applicants’ needs. Refers consumers to other needed and appropriate resources.
e. Verifies information and prepares cases for enrollment approval. Maintains accurate and complete case records.
f. Works co-operatively with other housing staff to insure service delivery. Participates in case conferences with the occupational therapist and others as required. Interfaces with other PCA programs, outside agencies and others to develop appropriate service plan with consumers when necessary.
g. Provides outreach to agencies and the community to promote appropriate utilization of PCA’s Housing Services.
h. Acts as a liaison between the Aging and Housing service networks using information about the benefits of home modifications and the variety of housing including home ownership, public housing, subsidized housing, assisted living and other supportive housing options.
i. Increases or maintains up-to-date information and knowledge of housing resources, modifications, and services to better serve SHARP consumers.
j. Other related duties as assigned.
