Home Care Case Manager
Greater Lynn Senior Services provides a wide range of social services for elders and people with disabilities – information, transportation, meals, advocacy, homecare, housing, and more—for one reason: to help those 60 and older in Lynn, Lynnfield, Nahant, Saugus and Swampscott live fuller, more independent lives, safely and with dignity.Â
The Home Care Case Manager has daily responsibility for developing, authorizing, coordinating, and monitoring community-based services to an assigned number of frail elders. The process begins with a comprehensive needs assessment, completed as part of an interdisciplinary team. Requires a thorough knowledge of available community services, an understanding of both formal and informal support systems, and an ability to interface and collaborate with those systems to ensure the elder’s needs are met. The position entails extensive coordination, follow-up, and feedback to clients and services providers, as well as recordkeeping incompliance with regulations

Qualifications:
-Â Â Â Â Â Â Â Â Â Â B.A. degree in Human Services or related field
-Â Â Â Â Â Â Â Â Â Â 1-2 years experience in Human Service (Human Services experience may be substituted for degree)
