HCBS ACP Assessor
SUMMARY
Â
This position will assess consumer eligibility for Home and Community Based Services (HCBS) programs. Most of the time will be spent conducting in-home assessments for consumers applying to enroll into HCBS Waivers or Programs. The Assessor will work to continuously uphold the Independent Living Philosophy and the mission of Liberty Resources, Inc.

MINIMUM QUALIFICATIONS:Â Â Â Â Â
Â
A Minimum of a Bachelors’ Degree obtained in a Human Services field from an accredited educational facility and one year of directly related work experience as a Supports/Service Coordinator or Case Manager conducting service assessments. Incumbent is required to successfully complete resource management training within the first 90 days of hire. Incumbent must have excellent communication skills (written and oral); Strong organizational skills and Computer literacy.
