Forensic Act Team ICM-CAC

The Forensic ACT Team is a specialized CTT Team that provides multidisciplinary clinical review and assessment of individuals actively involved in the criminal justice system who have co-occurring behavioral health diagnoses, with the purpose of facilitating their return to the community. Intensive Case Manager provides case management supports to all individuals assigned to his/her team as a part of the Community Support Program of Philadelphia.
Education and Experience: Bachelor’s Degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, education with 2 years related experience preferred; Bachelor’s Degree in nursing with 3 years mental health direct care experience; be a registered nurse. Prefer expertise in behavioral health and criminal justice systems, and must have CAC, with training in PCP, and familiarity with community resources.
Other Job Requirements: Must have a valid driver’s license, auto insurance and access to a vehicle; able to share office space and work as a part of a team; able to work evenings and weekends, and have a good knowledge of city transit system.
