Elder Case Manager: Haitian-Creole speaking a Plus!

ELDER CASE MANAGER  at a non-profit serving frail, low income elders Ability to Speak Haitian-Creole is a Big Plus!  

 

Ethos has served its diverse group of clients for almost 35 years. We are a stable organization of 100+ employees  working in very pleasant surroundings with family-friendly policies. As a Case Manager, you will conduct personal interviews to assess client needs; coordinate services with providers; act as an advocate for clients obtaining government and other services; provide short-term crisis management support and provide ongoing care management to clients.

 

DUTIES:

Maintain an ongoing caseload of clients.

1.      Conduct initial assessment of needs via personal interview and reassess at least every 6 months.

2.      Develop a care plan and coordinate services with provider(s) and informal/formal supports.

3.      Provide short-term support and intervention.

4.      Act as an advocate for clients in areas such as Social Security, Medicare, SSI, Medicaid, housing, legal services, fuel assistance, etc. Assist client in coordination of these services as necessary.

Maintain current information relating to each case.

1.       Keep files up-to-date including progress notes.

2.       Maintain all forms in compliance with regulations.

3.       Complete authorizations, re‑authorizations and changes in the service plan.

4.       Record and maintain statistical data.   

5.       Maintain appeals forms, notifying client of any changes in care plan. 

 

QUALIFICATIONS:

1.          Ability to speak Haitian-Creole, Spanish or Russian a PLUS!

2.          Bachelor’s degree in human services, social work, psychology, counseling, nursing or related field required. 

3.          Excellent interpersonal, communication and organizational skills. Sensitivity to elder issues.

4.          Intermediate computer literacy and word processing skills required.
