CASE MANAGER (ADOLESCENT)

PHOENIX HOUSES OF CALIFORNIA

 

Phoenix House, a non-profit organization, is the nation’s foremost provider of comprehensive, alcohol and substance abuse treatment for adults and adolescents. Since 1967, more than 100,000 drug-troubled individuals have received treatment at our adult residential centers, our adolescent Phoenix Academies, outpatient and day programs and programs in prisons, drug courts and homeless shelters.

 

Case Manager (Adolescent)

 

Reporting to a Senior Counselor or Clinical Specialist in an adolescent facility, the Case Manager is the designated advocate for each client. It is their role to interface, network and communicate with referral agencies and community resources, as well as parents and other concerned family members; and to ensure that comprehensive quality clinical services are consistently and effectively delivered to each client assigned to their caseload in collaboration with the multi-disciplinary team and based on the client’s treatment and needs and services plans

 

Responsibilities:

 

Ø  Collaborates with members of the multi-disciplinary treatment team for the delivery of a full spectrum of clinical services.

 

Ø  Assists clients in meeting individual treatment plans, goals and objectives for assigned cases.

 

Ø  Provides individual counseling services.

 

Ø  Utilizes appropriate crisis intervention and de-escalation techniques in response to problems and/or behaviors which place the child or other residents in danger.

 

Ø  Communicates with parents and other approved family members.  Encourages and facilitates a family involvement in parent education, weekly visits, family recreation days, and other Phoenix House events. 

 

Ø  Interfaces with referral agencies such as Juvenile Probation, Department of Children and Family Services, and Department of Mental Health.  Completes and submits Needs and Service Plans, Quarterly Reports, and Discharge Plans on a timely basis.

 

Ø  Acts as a liaison between Phoenix House and Vocational/Education counseling staff to ensure quality educational and vocational services are provided to the client. 

 

Ø  Communicates  with the Nursing Department and consulting physicians to ensure quality medical services are provided to the client on a timely and as needed basis.

 

Ø  Completes and maintains all documentation of services delivered which includes individual records, group notes and general communication logs.

 

Ø  Attends and participates in weekly staff meetings, community meetings, and program and family gatherings.

 

Ø  Facilitates routine case conferences and maintain records of these using prescribed documentation guidelines.

 

Ø  May perform other related duties as assigned.

 

Supervision:

 

No supervision is exercised.

 

Knowledge of:

Basic Phoenix Houses of California rules, regulations, practices and policies

Case Management Techniques and Best Practices in Colloborative Team Case Work

 

Ability to: 

 

Provide leadership and prioritize projects

Communicate with all members of internal and external multi-disciplinary team

 

QUALIFICATIONS:

 

Education:

 

Baccalaureate degree (BA or BS) in Social Work from an accredited college or university OR completion of a BA/BS degree in a behavior science with active enrollment in an M.S.W. program OR a Master’s in Counseling.

 

Experience:



Two years (paid or intern) experience providing counseling or related services to adolescents.

 

Salary:

 

$32,000-$40,000 with full benefits

 

SPECIAL NOTES

 

1.Must possess a valid California Drivers license/ may be required to secure a Class B license

2.Excellent computer skills, writing and verbal communication skills

2.Non-Exempt from FLSA.

3.Incumbents in these classes will be required to transport residents to and from appointments.

4. Incumbents will be required to represent the treatment team recommendations in court.

5. Applicants not in possession of substance abuse counselor certification must register within first 6 months of employment.
