This position, focused on telephonic assessments, offers plenty of growth, job variety and a reliable M-F weekday schedule. If you’re ready for a refreshing change from bedside care, we urge you to come and apply your clinical skills to the following responsibilities:Managing high risk cases for patients with chronic illnesses Making outbound calls to patients to discuss their health risks Engaging members via telephone, and working to affect their behavior and mitigate health risks Researching and reviewing patient information Running daily queries to determine appropriate members to be contacted Documenting all case management activities in our internal system Only licensed Tennessee RNs will be considered for this role, and solid clinical experience is also needed, ideally in home care, geriatrics, chronic illness and/or long-term care. Case management experience is a must. Medicaid, Medicare, telephonic triage and Tenncare population experience is preferred. CCMs would be best equipped for this role. Milliman criteria knowledge would be an asset in this position. PC and typing proficiency is a must.
