RN - Case Mgmt, Utilization Review, Disease Management, Health Coaching
LPN -Disease Management Program Intake Consultants

Must have an active Virginia RN or LPN license with 5 years’ clinical practice experience and strong computer skills. Bilingual (English-Spanish) skills area plus as well as specialties in Oncology, Maternity, Cardiovascular, Rehab Pediatrics and NICU. Certified Diabetic educators and Certified Case Managers are preferred.
