RESPONSIBILITIES: - Our Registered Nurses work as part of a team to facilitate quality health care. - The ideal candidate will possess the many skills necessary to expertly assist members in self-management of chronic disease processes, with an emphasis on health education and behavior modification for modifiable risks. - Collaboration with the member and the attending physician is an integral role for the Disease Management nurse. - We utilize criteria-decision support tools to determine medical appropriateness for identification and enrollment in specific Disease Management programs.ADDITIONAL JOB INFORMATION: - The Nurse Consultant will make proactive outbound calls to members who have been identified as potential candidates for a specific Aetna Health Connections Disease Management program. - This is a challenging opportunity for highly skilled Registered Nurses to influence healthcare outcomes by interacting with members and their physicians in our comprehensive Disease Management model. - The work hours for this position are 10:00 am 8:00 pm. Unit business hours are from 8:00am-11:00pm. Required Skills: REQUIRED SKILLS: - A Registered Nurse with current state license and a minimum of three years recent clinical nursing experience. - Ability to exercise independent and sound judgment in decision making, utilizing all relevant information to effectively influence behavior. - The ideal candidate needs to have strong verbal and written communication skills. - The Nurse Consultants needs organizational skills, effective time management, ability to manage multiple priorities, and strong teamwork skills. - Candidate must possess strong computer and keyboarding skills. - Knowledge of managed care, eTUMS, Healthwise Knowledgebase and Disease Management preferred. - The Disease Management Nurse applies the standard elements of assessment, planning, implementation, coordination, monitoring and evaluation and will use state-of-art information tools. - Nurses who are bilingual in Spanish, is a plus. - Work from home a possibility for qualified employees after 1 year of service.
