They make it possible for members to get the right health care treatment for their needs and for Aetna to keep its competitive edge.RESPONSIBILITIES: - We are seeking dynamic, proactive Nurse Consultants for the Aetna Health Connections Disease Management Program that can work evening shift (2:30pm-11pm). - Aetna Health Connections will provide innovative and individualized clinical programs, information and support for total health management to help members achieve their optimal state of health. - Our disease management programs will be able to view Aetna members holistically considering multiple diseases or conditions across all benefit plans and deliver individualized programs based on their unique needs and preferences. - The Nurse Consultants will make proactive outbound calls to members who have been identified as potential candidates for a specific Aetna Health Connections Disease Management program. - The Aetna Health Connections Disease Management Nurse applies the standard elements of assessment, planning, implementation, coordination, monitoring and evaluation and will use state-of-art information tools. - Nurse consultants are required to be on an 800-queue line. The ideal candidate must be willing to be flexible to allow for scheduling changes, if required, due to business needs. Required Skills: REQUIRED SKILLS: We seek the following qualifications: - Registered Nurse with current state license and a minimum of three years recent clinical nursing experience. - Ability to exercise independent and sound judgment in decision making, utilizing all relevant information to effectively influence behavior. - Strong verbal and written communication skills. Organizational skills, effective time management, ability to manage multiple priorities, and strong teamwork skills. - Candidate must possess strong computer and advanced keyboarding skills. - Knowledge of managed care and Disease Management preferred. - Bilingual in Spanish is a plus.
