Case Management Nurse Consultatnt - Disease Management

POSITION SUMMARY
The nurse consultant utilizes clinical skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options and services to facilitate appropriate healthcare outcomes for members.
More specifically, Case Management (CM) is defined as a collaborative process that assesses, plans, implements coordinates, monitors and evaluates options and services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes.
The CM process includes: assessing the member's health status and care coordination needs, inpatient review and discharge planning, developing and implementing the CM plan, monitoring and evaluating the plan and involving the Medical Director as indicated and closing the case as appropriate when the member has met discharge criteria.

ADDITIONAL JOB INFORMATION
The working hours are Monday to Friday 8 to 5 pm with 1 to 2 evening shifts a week from 12:30 pm to 9 pm.


Required Skills:
LICENSES AND CERTIFICATIONS
- Registered Nurse (RN) is required
FUNCTIONAL WORK EXPERIENCE
- Disease management, Health Coach, Health Advocate
REQUIRED SKILLS
- Registered Nurse with 3-5 years of clinical experience
- Excellent verbal and written communication skills required to provide patient education
- Assess members clinical care needs
- Develop patient care management plans
- Integrate empathy framework in interactions with members
- Interpersonal Communication
- Leveraging Technology
- Provide members with health care information to drive member understanding of his-her health condition(s) and possible options

- Proficiency in use of computer applications and typing skills (35 wpm or more) highly recommended
