Stoke Center Coordinator
Job Description: The Mount Sinai Stroke Center Coordinator (SCC) is a registered nurse or nurse practitioner responsible for the coordination of systems and services required to maintain an organized, interdisciplinary, patient centered, The Joint Commission (TJC), and New York State Department of Health ( NYSDOH) designated Stroke Center. The scope of this position covers both the Mount Sinai Manhattan (MSM) and Mount Sinai Queens (MSQ) campuses of the Mount Sinai Hospital. Responsibilities include a variety of clinical and administrative functions dedicated to ensure continuity of care, patient satisfaction and compliance with nationally recognized, NYSDOH, TJC and MSH Standards; as well as an evaluation of the systems designed to evaluate the effectiveness of implementation on both campuses Reporting: The SCC will report to the Mount Sinai Hospital Stroke Center Director and to the Director of Performance Improvement at the Manhattan Campus with a dotted line to the Director of Quality at the Queens Campus. The SCC also works closely with Hospital Operations at MSM and MSQ, to ensure compliance with TJC, DOH and other related activities. Specific SCC responsibilities will include but not be limited to:1)Coordinating all aspects of quality assurance activities of the Stroke Center, together with the identification of patients for inclusion in the hospital stroke center database, data abstraction, collection and the demonstration of performance improvement required by TJC and NYSDOH to maintain their Stroke Center designations. 2)Coordinating the ongoing effort of implementing performance improvement initiatives, organizing, implementing and evaluating the quality and performance improvement activities required by TJC and NYSDOH to maintain their Stroke Center designations. 3)Preparation of Stroke Center Quality and other related reports in consultation with the Department of Quality Assessment and Performance Improvement. 4)Coordinating the quarterly Stroke Center Performance Improvement Committee meetings mandated by New York State Department of Health.5)Scheduling and overseeing all Stroke Center related meetings, as well as the ongoing tasks of these Committees and subcommittees, including but not limited to:a.recording and distributing minutes and meeting materials;b.preparation and distribution of agendas and related documents;c.maintaining Stroke Center Policies and Procedures (paper and web-based) and Performance Improvement documents 6) Training professional nurses and other staff in areas related to the clinical requirements and educational efforts of the Stroke Center. This will include staff in the Gustave Levy Acute Stroke Unit, NSICU and ED, as well as on the neurology floor and staff at MSQ involved in the care of stroke patients. Educational activities are inclusive of but not limited to training nurses on the application of the NIH stroke scale and simulations of stroke codes. 7)Maintain credentials and CME credits for the fellows who pass through the Mount Sinai Stroke program. 8)Organizing, publicizing and participating in community stroke education and screening activities and other Stroke Center activities such as the NYS DOH and TJC mandated CME stroke conferences for professionals, and other special events.Job Qualifications: * Registered Nurse with Masters Degree or Nurse Practitioner* Certified Neuroscience Registered Nurse (CNRN) preferred* 3-5 years related neuroscience nursing experience* Experience with data collection, analysis, and report writing* Experience with Microsoft Office and other productivity applications required
