Job Purpose:
Supports residents and families by developing social work service policies; identifying service requirements; providing services; directing staff.

Duties:
* Accomplishes social work services human resource strategies by determining accountabilities; communicating and enforcing values, policies, and procedures; implementing recruitment, selection, orientation, training, coaching, counseling, disciplinary, and communication programs; planning, monitoring, appraising, and reviewing job contributions; planning and reviewing compensation strategies.

* Develops social work services organizational strategies by contributing social work services information, analysis, and recommendations to strategic thinking and direction; establishing social work objectives in line with organizational objectives.

* Guides healthcare team actions by preparing and disseminating social-environmental policies, procedures, and protocols; coaching providers; advocating for social work, discharge planning, and resident life initiatives.

* Avoids legal challenges by understanding current and proposed legislation; advising on the impact of legislation; enforcing regulations; recommending new policies.

* Builds rapport with community resources by visiting with service providers; providing consultative services; resolving conflicts; expediting arrangements.

* Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

* Enhances social services and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

Skills/Qualifications: Long Term Care Social Work Experience Preferred.
 Handles Pressure, Confidentiality, Resident Relationships, Legal Compliance, Internal Communications, Quick Study, Community Outreach, Analyzing Information, Process Improvement, Positive attitude and Team Player approach.  Resident Centered Care.
