Home Health Services is a nonprofit, community-based home care agency with six (6) branch locations in the Dallas-Fort Worth, TX area and due to recent expansion, a branch office in Houston, TX. Accredited by JCAHO and certified by Medicare, Home Health Services has been meeting the needs for home health care specialized in serving the senior population since 1974.

 

We are currently seeking qualified candidates for a full time Physical Therapist Assistant for our Arlington, TX office.

 

Physical Therapist Assistants are responsible for providing physical therapy services which are planned, delegated, and supervised by the Physical Therapist, under the supervision of the Team Manager or Branch Manager and in complies with the regulations and standards as established by the State Board of Physical Therapy and patient care policies and procedures of the agency.

 

Qualifications

§ Graduate of a 2-year college - level program approved by the Council of Medical Education of the AMA in collaboration with the American Physical Therapy Association or its equivalent.

 

§ Licensed as a physical therapist assistant in the State of Texas.

 

§ Eligible for membership in the APTA.

 

§ Two years of appropriate experience as a physical therapist assistant.

 

§ Home health experience is preferred.

 



RESPONSIBILITIES AND DUTIES

 

§ Provides Skilled Rehab care as outlined in the Rehab Care Plan.

 

§ Charts those services the staff P.T.A. has rendered to the patient, changes noted in the patient's condition and/or family and home situations, makes revisions in the rehab care plan as needed, assesses patient care and progress documentation of the preceding week to be provided to the appropriate agency designated department within the time tables set by the agency, but no later than five (5) days from the date care is given.

 

§ Submits all required patient documentation according to agency timetables.:

 

§ Prepares and presents patient record to the UR Committee at planned intervals.

 

§ Evaluates the effectiveness of her/his rehab service to the individual and family.

 

§ Consults with the attending physician concerning alteration of the plan of treatment.

 



§ Obtains orders and submits for referrals to other appropriate disciplines after discussing the need for such services with the Physical Therapist.
