Senior Care Manager-RN, Oncology

  POSITION SUMMARYUtilization of professional nursing skills via telephonic communication for participants in condition management and lifestyle programs, in order to achieve program goals and positive health outcomes for a given population. Uses company experience and professional knowledge to support and enhance the professionalism and effectiveness of Care Center employees.PRINCIPAL RESPONSIBLITIES include the following.  Other duties may be assigned.Ø Ability to effectively support, initiate and participate in activities that enhance the overall impact and effectiveness of the Care Center.o  Acts as a super user of all technology applications used to support the Care Manager role.o  Precepts and mentor new hire Care Managers and other Clinical new hires.o  Assumes lead role in special projects that utilize specialized or individual skills and/or knowledge.o  Acts as a key employee to perform system demonstrations and readiness reviews for new and potential clients. Ã˜ Ability to perform clinical assessment, planning, intervention, and evaluationo  Assesses participants with regard to chronic condition(s), co occurring and/or complicating factors, educational needs, abilities, and provider treatment plan.o  Develops defined, goal based plans with participantso  Intervenes based on participant acuity level and as indicated to effect movement toward goals and behavioral change o  Monitors participants via scheduled and unscheduled contactso  Evaluates the status of the factors assessed, goals, plan, interventions, and modifies as appropriate/indicated.  Ã˜ Ability to use various forms of communication and to adapt communication to individuals from a wide range of backgrounds.o  Builds trusting, respecting, and empowering relationships with participantso  Presents information in a simple manner and confirms participants’ understandingo  Communicates professionally with participants’ health care teams/ providers when indicated to enhance, update, and/or promote and verify appropriate treatment plans. Ã˜ Ability to educate participants about their specific conditions with particular attention to current guidelines:o  Teaches participants about causes, warning signs, and consequences of condition implicationso  Advocates for delivery of care consistent with evidence based standards Ã˜ Ability to promote effective health behaviorso  Encourages participants to adhere to appropriate treatment planso  Empowers participants to actively manage their health and their diseases/conditions through appropriate key self care behaviors.o  Uses effective behavioral change practices, promotes appropriate behavioral and lifestyle changes Ã˜ Ability to promote participants’ engagement of support and resources as appropriateo  When appropriate and with participant permission, aids family caregivers by providing information/education. o  Refers and encourages participants’ utilization of available community and other resources. Ã˜  Ability to support participants’ safetyo  Educates participants about risks related to health conditions and lifestyle behaviorso  As appropriate, encourages participants to regularly review treatment medications and side effects with   their health care providers. Ã˜ Ability to support and promote participant life quality leading to improved clinical outcome(s).o  Building on participant abilities, reinforces participants’ engagement in making active decisions and personal choiceso  Supports the highest level of participant-defined quality of life and well beingo  As and when appropriate, educates participants on end of life issues, including condition-specific treatment guidelines when available, advance directives, and palliative care.EDUCATION/EXPERIENCE Ã˜  Minimum of three years experience in appropriate clinical setting(s). CERTIFICATES, LICENSES, REGISTRATIONS  Ã˜ Registered Nurse with valid, current license in state of residence/employment and the ability to be licensed in other states per Matria’s Nurse Licensure Policy.TECHNICAL SKILLSØ Proficient computer skills in Windows and Internet use.Ø Ability to chart and follow designated workflow(s) in an electronic environment. COMMUNICATIONS SKILLSØ Strong verbal and written communication skills required to meet superior customer service and satisfaction levels.Ø Excellent interpersonal skills and ability to function as a member of a multi-disciplinary team.
