SOCIAL WORKER MSW-Per Diem

Location: GLENDALE Status: Per Diem, Employee Job Category: Medical/Health Reference Code: 58073 Job Description This MSW/LCSW preferred position is per diem and includes multiple roles including coverage for medical social work, discharge planning, weekday or weekend on call. In the medical social work capacity the msw will Provide psychosocial assessments, identifies issues/problems such as social, emotional, maturational, cultural, financial and/or legal followed by implementation of suitable interventions. Provides appropriate assistance, referral, counseling to enable patients/families to utilize medical care in order to achieve their optimum levels of health. Requires a MSW degree and experience in a hospital setting. In the discharge planning capacity the social workers will assist RN case managers with placement of patients to an alterantive level of care and doing initial discharge planning asessments. On call shifts are from Monday at 4:30- Friday 8am (with the exception of 8-4:30 Monday-Friday when social work is on site) and Friday at 4:30-Monday 8am. This position is VERY flexible and we will work around your availability. Available work hours are Monday- Saturday day or evening as well as on call shifts. You are required to work a minimum of one shift per month, however you can choose the day and shift and if it is on call or in the hospital. For Prevention - For Treatment - For Life.
