Registered Nurse (Weekend Program) - Home Hospice

Job Description SIGN-ON BONUS INCREASED UNTIL 8/1/08!!!


Our Aurora Visiting Nurse Assocation provides vital support to those Hospice patients who want to remain in their homes. Imagine:

Providing end of life support to these patients while working as part of a multi-disciplinary team to care for these patients in their time of greatest need.
Performing the initial hospice admission assessment and establish the initial plan of care.
Communicating with physicians and other team members to confirm, correlate and update patient plan of treatment and provide physician with a status report on a regular basis.
Providing on going assessment of patient response to treatments, medications and teaching and applies interventions as appropriate.

This position will provide support in the Milwaukee county area.


General Qualifications

Associate's Degree in nursing.
Minimum two years of home health/hospice experience or demonstrated competency in direct home health or hospice care.
Current licensure to practice as a Registered Nurse in the state of Wisconsin.
Proficiency in clinical, communication and organizational skills with the ability to work independently and make sound judgments.
