Job Description Sign On Bonus Offered!!!!
Our Aurora Visiting Nurse Association provides vital support to patients in their homes.

Providing support to patients while working as part of a multidisciplinary team.
Performing the inital admission assessment and establish the inital plan of care.
Communicating with physicisans and other tema members to confirm, correlate and update patient plan of treatment and provide physician with a stauts report.

Provide ongoing assessment of patient response to treatments, medication and teaching and applying interventions as appropriate.

General Qualifications
RN License

Valid WI Drivers License and reliable transportation

2 years of acute care experience

Home Care experience is preferred
