Berks Visiting Nurse Association is currently seeking a full time Registered Professional Nurse to perform nursing services to our hospice and palliative care clients in their home of choice. 

As a Visiting Registered Nurse, you will provide one-on-one skilled nursing care to clients requiring intermittent professional service, perform a complete health assessment, effectively analyzes situations, develop and implement goals and Plans of Care, evaluate effectiveness of Plans, and advocate for clients in obtaining needed services.  You will also provide knowledge and comfort to the family members in an effort to help them cope.

Our Registered Nurses are responsible to plan with the family, physician, and Interdisciplinary Team appropriate care that is feasible within the resources of the family, interpret to the client and family the implications of the diagnosis, prognosis and treatment, observe, evaluate and report to physician any changes, review all medications to identify possible reactions, side effects, allergies and report to the physician.

Additional responsibilities include exchanging information with other health and referral sources as needed, completing documentation tasks, identifying the case load for the day, adjusting the daily schedule to accommodate work, keeping the Manager informed of major changes in client and level of care changes, and initiating Hospice Plan of Care with the team.

This position performs assigned tasks in a geographic area covered by the Berks Visiting Nurse Association, performs ``On Call`` activities on a rotating basis, participates in in-service programs and selected research projects and helps select clients for student affiliation.

REQUIREMENTS
Graduation from a State accredited Nursing Program and valid license to practice in the State of Pennsylvania.
Knowledge of Hospice specialized care demonstrated by work history in one of the following:
1) One to 2 years of oncology and/or psyche nursing
2) Past experience in Hospice homecare of at least 1 year
3) Care of terminally ill clients in an institution (i.e., hospital, ECF) within the past year.

Must have a valid Pennsylvania Driver’s license, automobile insurance, available automobile during working hours and be able to drive in all types of weather, and be able to stand, walk, sit and lift without weight restriction.
