Registered Nurse - Case Mix Director - MDS Coordinator

Registered Nurse: Assumes responsibility and accountability for the provision of resident care according to professional standards of nursing practice in compliance with state and federal laws and regulations. Utilizes resources in an efficient and economic manner to ensure each resident achieves the highest practicable physical, mental, and social well-being. Performs any and all professional nursing duties as determined by qualifications and training.EDUCATION: Registered Nurse with current active state license and a graduate of an accredited school of nursing.EXPERIENCE: 1-year professional experience in a long-term care setting. Supervisory experience preferred. SKILLS: Strong interpersonal, leadership, organizational, and clinical skills.Case Mix Director: Responsible for coordinating assessments for each resident including RAI, RAPS and Care Plan. This includes all admission, quarterly, annual and significant change assessments as well as any Medicare assessment. Responsibilities include maintaining accurate and timely assessments, meeting all automation requirements as defined by regulations, facilitating a functioning interdisciplinary team, accurate reporting of RUGS to all departments, maintaining Roster and Census reports and monitoring of all reports generated by this department. EDUCATION: Graduate of an accredited school of nursing. Current and active license as a Registered Nurse or Licensed Practical Nurse. EXPERIENCE: 1 year of professional nursing experience in a skilled nursing facility required. 1 year of administrative and RAI experience preferred.SKILLS: Strong computer, interpersonal, leadership, organizational, and clinical skills.MDS Coordinator: Assist in assessing each resident’s condition annually, quarterly or when significant change in condition occurs the utilizing the RAI process; Responsible for coordinating the efforts of the interdepartmental team in developing a comprehensive resident care plan in an effort to best meet the needs of each individual resident. EDUCATION: An active Georgia Licensed Nurse.EXPERIENCE: 1 year in long term care with administrative experience preferred.SKILLS: Strong computer, interpersonal, leadership, organizational, and clinical skills.
