Is a 118-bed facility designed and licensed as an acute care hospital. They are highly respected in the community for their clinical programs to care for post acute and ventilator patients. Harmon has a state of the art acute rehabilitation unit and sleep center. They are located 1 mile from the strip and close by to University of Nevada Las Vegas.



Henderson Healthcare Center 1180 E. Lake Mead Pkwy Henderson NV 89015

 

Is a 266-bed facility that offers subacute, skilled nursing and long term care services for long and short term patients. They have a 10 bed ventilator unit and respiratory staff to meet the needs of residents who require more specialized care. They are located near Catholic Charities Hospitals, St.Rose Delima and St.Rose Siena.



Horizon Specialty Hospital 640 Desert Lane LAS VEGAS NV 89106

 

Is a 45-bed long term care acute care hospital (LTACH).They are known for their clinical programs  in the community offering a premier ventilator program staffed with highly qualified respiratory therapist and a respected wound care program. Near University Medical Center and Valley Hospital.



Mountain View Care Center 601 Adams Blvd BOULDER CITY NV 89005

Is an 85-bed facility that offers skilled nursing and long term services. They are experienced in eldercare services and short and long term rehabilitation to assist the resident in reaching their maximum level of independence. Located about 5 miles from Hoover Dam.



North Las Vegas Care Center 3215 E. Cheyenne Ave. NLV 89030

 

Is a 178-bed facility that offers skilled nursing and long term care services along with a 40 bed locked Alzheimer’s unit. The facility sits across the street from the Community College of Southern Nevada Campus.



Vegas Valley Rehabilitation Hospital 2945 Casa Vegas LV  89109

 

Is a 100-bed facility offering subacute, rehabilitative and skilled nursing services. They have an excellent rehabilitation program with stat of the art equipment for both short and long term patients. Near Sunrise Medical Center.

Requirements

Infection Control (RN req)

Monitors hospital for possible infection outbreak. Implements and maintains plans and procedures concerning control measures as set forth by the infection control committee. Identifies and initiates isolation protocols and informs the appropriate state and county organizations of any reportable diseases.

 

Qualifications*

 

Requires an associate's degree and is certified as a registered nurse with at least 2 years of clinical experience. Familiar with standard concepts, practices, and procedures within a particular field.

 

 

MDS (RN req) 

The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM). These duties are to be performed in a timely manner consistent with Centers for Medicare and Medicaid (CMS) guidelines and Trans Health standards. This individual also establishes and maintains positive relationships with all members of the healthcare team.

Essential Functions (summary):
Maintains current MDS status of each patient/resident, according to state and federal guidelines as required under the Omnibus Budget Recomciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.
Schedules patients/residents for appropriate OBRA, Medicare PPS and or Medicaid required assessment.
Implements system to monitor for changes in patient/resident condition.

 

Qualifications*


Ideal candidate must be a skilled communicator, director and motivator. Able to organize and prioritize many tasks effectively.
RN with unencumbered license to practice as an RN.
Able to react to emergency situations appropriately when required.
Must be able to read, write, speak and understand the English language
Must be able to relate information concerning a patient/resident’s condition.

 

RN Case Manager

Essential Function:

To facilitate communication and coordination between all members of the Healthcare team to promote quality of care, maintain cost effectiveness and achieve optimal patient outcomes. Case Management is a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes.

 

Qualifications*
Ideal candidate must be a caring individual dedicated to providing high quality care to residents in the care of facility
RN with current Nevada license.
Recent clinical experience (3-5 years) or case management experience (1-2 years) required.
Ability to handle a variety of projects/tasks simultaneously and able to plan, coordinate, implement and evaluate required in a hospital or alternate level of care.
Strong written and verbal skills required.

 

Wound Care Nurse (LPN)

Henderson Healthcare Center is a 266 bed facility offering subacute, skilled nursing and long term care services for both long and short term patients. We are searching for talented RN’s. Henderson Healthcare Center focuses on and is dedicated to providing high quality care.
The Wound Care Specialist coordinates and facilitates aspects of care related to wounds, ostomies and incontinence throughout the facility. Essential Functions (summary): Provides consultation and direct assistance in the management of patients with altered skin integrity (pressure necrosis, wounds, fistulas, etc). Provides inservice programs as needed to assist nursing, rehabilitation and medical staff to maintain knowledge and competence in wound care. Collects, analyzes and documents information and statistics related to wounds in the facility. Performs clinical evaluations for potential wound admissions. Participates in product research relative to ET practice.

 

Qualifications*

Ideal candidate must be a skilled communicator, director and motivator. Able to organize and prioritize many tasks effectively.
Licensed RN with unencumbered license to practice

Completion of an Enterostomal Therapy Nursing Education Program preferred

1-2 years wound care experience in an acute care setting required. Willing to train the right person.

 

Performance Improvment Coordinator(RN)

Improvement Coordinator for our 118-bed Acute care hospital facility. We are located at 2170 East Harmon Ave Las Vegas NV 89119.

Coordinators, evaluates and assists in the development of performance improvement programs at facilities located in the Southern Nevada region.

Participates in the development of strategies for performance improvement and communicates results of activities.

Performs reviews of quality indicators of facilities’ monitoring programs.

Provides support to facilities’ monitoring and evaluation programs.

Assists with the development, implementation and evaluation of the facilities’ performance improvement plan.

Reviews reports from facilities to ensure compliance with monitoring and reporting frequency as outlined in the facility performance improvement plan.

Assists to development and implementation of processes to continually asses and improve patient/resident outcomes.

Participates in Performance Improvement Committees/Teams.

Performs other duties as assigned.

Qualifications*

Current RN license in the state of Nevada.
Minimum of 3 years clinical nursing experience.
Two years experience on quality management/improvement or comparable experience in Utilization Review, Case management.
Must be able to read, write, understand and fluently speak the English language.
Must possess the ability to make independent decisions when necessary.
Must be able to effectively communicate appropriate information to all parties concerned as needed.
