RN/Team Manager in Northern Virginia
The RN/Team Manager is the leader of the Patient Care Team whose functions include: Supervising, evaluating, and coordinating the various component members of the interdisciplinary team. Responsibilities include continuity of care from admissions to discharge or transfer to bereavement. Ensures that services are rendered as integrated components of the interdisciplinary plan of care. Reviews patients for appropriateness and eligibility. Monitors the type and level of team services provided to assure appropriateness and responsiveness to patient and family needs and expectations. Manages the team and assures coordination of services to facilitate patient’s needs. Chair team meetings, assuring interdisciplinary input into the update and revision of the plan of care in concert with the changing needs and expectations of the patient and family. Assure staff competence and performance levels by making field visits, evaluating documentation and providing team based in services. Acts as a resource and mentor to staff. Reviews and approves payroll, mileage and additional expenses. Performs chart reviews and participates on the outcomes management and annual program review. Analyzes customer service issues on their team to identify causes and works with individual team members to improve performance. Controls and is accountable for productivity, labor and all related patient care costs with respect to the budget. The Team Manager also serves as a patient advocate and coordinator for other social services and health care providers in the community.
