The diabetic nurse educator works closely with the endocrinologist and Fallon Clinic Primary Care physicians to provide education and monitoring services to the at risk diabetic population. Job Responsibilities:
Provides evaluation and education for new diabetic patients and follows their progress until they are self sufficient.
Provides care in the individual and group setting.
Works with Primary care physicians throughout the system to identify patients who require educational and monitoring services for their diabetes.
Works closely with the staff endocrinologist to identify patients in this practice that require education and monitoring services.
Manages a caseload of patients independently.
Assesses patients for their learning needs, follows them appropriately and documents in the patient medical record.
Keeps abreast of new developments in the area of diabetes care. Maintains certification.
Provides staff education for nurses at various sites as requested.
Collaborates with the Practice Manager in relation to time utilization and program development.
Utilizes the curriculum developed for the care of diabetes patients.
Provides professional and courteous care to patients consistently.
Attends appropriate meetings.
Education, License, Certification Requirements:
Graduate of an accredited School of Nursing.
Currently licensed as an RN in the State of Massachusetts.
Basic Life Support (BLS) required.
A CDE is required for this position.
Experience and Technical Skills Required:
Experience with evaluation and education of diabetes preferably in an Endocrinology Practice.
Group and individual education experience required.
Supports Endocrinologists Practice to optimize patient care.
Pump experience is preferred.
