UniHealth SOURCE (Service Options Using Resources in Community Environments) is the access to a state program designed to provide elderly disabled and low-income patients with a wide range of long-term health services, either in the home or in the community. Responsible for screening clients and referring to provider for level of care needs. Experience with home health, mental health or long-term experience preferred. SKILLS: Excellent oral and written communication skills. Must possess good assessment and documentation skills.
