We are seeking a highly motivated, results-oriented full-time or part time Home-based Independent Contractor (1099) RN Case Manager. The company is a unique medical consulting and cost containment firm working for ERISA self-funded health plans, employer groups, third party administrators, Managing General Underwriters and carriers across the country.
Responsibilities:
Case Managers facilitate clinical solutions and coordinate cost management opportunities for members with short and long term medical needs. Following national standards and best practices, the key responsibilities include:
· Assess patient / member situation and develop a comprehensive, action-oriented and time specific medical intervention plan to address the needs and requirements of all parties.
· Implement, coordinate and monitor the plan to assure successful outcomes including clinical, financial, quality of life and client satisfaction.
· Create periodic Case Management reports.
· Functions as an effective liaison between patient, third party payer, reinsurance carrier and/or other customers, while implementing targeted interventions.
· Functions as a member of a multi-disciplinary medical team, including social workers, dieticians, pharmacologist and physician consultants.
Requirements:
·Registered Nurse License
· Minimum 3 – 5 years Case Management or related experience required
· Excellent verbal and written communication skills, as well as attention to detail, required
· Proficient in Microsoft Word
Preferred:
· BSN
· Oncology, Renal, Critical Care or Chronic Illness experience
· Certified Case Management Certification or Certified Nephrology Nurse
