Experienced RN Case Manager needed for busy Home Health office in Shreveport! Position Requirements: Responsible for planning, coordinating, providing and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge.  Utilziing physician's orders and professional skills, the Case Manager RN develops and implements a Home Health plan that meets each patient's specific need and is in compliance with Federal and State regulations, reimbursement guidelines, as well as, American Nursing Services Policies and Procedures.Qualifications: RN license in state of LA Must have 1 yr of experience in Home Health, with at least 6 months of Case Management experience in Home Health Benefits: 75% employer paid health benefits 3 weeks PTO after 90 days 401K with quarterly enrollments $500 per year for tuition reimbursement short & long term disability, life insurance 26 year old national company, founded by a Registered Nurse To Apply: please contact Amy Gatti, Corporate Recruiter at agatti@american-nurse.com or fax resume to 985 951-8285.  Visit website at www.american-nurse.com for more information.  EOE
