These positions, focused on telephonic assessments, offer plenty of career growth, job variety and a reliable M-F weekday schedule. General responsibilities of a RN Case Manager include: Managing high risk cases for patients with chronic illnesses Making outbound calls to patients to discuss their health risks Engaging members via telephone, and working to affect their behavior and mitigate health risks Researching and reviewing patient information Running daily queries to determine appropriate members to be contacted Documenting all case management activities in our internal system The requirements for this role are as follows:Current, unrestricted RN license in the state of TN 3 + years clinical experience PC proficiency, to include MS Word and the ability to type and talk at the same timeAdditional assets include:Medicare / Medicaid (TennCare) knowledge/experience Experience with home care, chronic illness and/or long-term care Telephonic triage experience Experience in case management Knowledge of Milliman criteria Ability to adapt to a fast-paced, ever-changing environment Ability to work with a diverse member population CCM designationWhen you become part of AmeriChoice, you also become part of something larger—a Fortune 100 leader with a broad spectrum of services and products—while still working for a company where you can see and feel your impact.
