RN Care Manager

  Position Summary:To provide direct skilled nursing care and care management for home care patient’s as part of interdisciplinary team under physician’s orders.Certificates and Licenses Required: Current state license as a Registered Nurse Valid driver’s license and automobile liability insurance Current CPR certificationKnowledge and Skills: Minimum of two years nursing experience. Previous home care/hospice experience preferred Possesses a thorough understanding of home care regulations and agency policies and procedures. Possesses strong nursing assessment and decision making skills. Possesses excellent verbal and written communication skills.
