Primary responsibilities include: direct care to home healthcare patients in their home, coordinating patient care, collaboration w/ other health care professionals, working w/ the interdisciplinary team members to provide support & care to patients, provide accurate & timely documentation, provide support & education for families based on the home healthcare philosophy of care.
