RN CASE MANAGER/COORDINATOR
Northeast Valley Health Corporation is one of the nation’s largest community health centers. We are a nonprofit organization that is community owned and controlled. We are here to provide health care services to adults, children, adolescents and families who are medically under-served, regardless of their ability to pay. We provide services in comprehensive primary care centers, on school campuses, in homeless shelters and in community-based locations in areas of high need. We provide culturally competent, quality health care with dignity.
The RN Case Manager will provide case coordination and management for high risk women in the SPA 2 target areas during the perinatal, intrepartum and post partum periods. The RN Case Manger will assist high-risk pregnant and immediate post-partum women develop and implement a comprehensive care plan to meet the client’s physical, functional, social, psychological, environmental, and financial needs. Will conduct home visits to provide case management services to women enrolled into Best Babies Collaborative.
BASIC QUALIFICATIONS:Current License to practice in CaliforniaWomen’s Health knowledge/experience preferredBilingual English/Spanish requiredAbility to pass a basic clinical and math skills screening examDependable and reliable vehicle liability insurance
