As part of the Telecare family, Transitions North is an Assertive Community Treatment (ACT) program for individuals with severe mental illness and/or medical fragility. Services are provided to Clients living in the community (homes, transitional housing, hospitals, etc). -- building trust, establishing a partnership, and striving for success.

 

As an RN/LVN or LPT, you will:

 

Ø  Provide safe and appropriate administration of medical treatment (including medications)

Ø  Provide documentation of response to treatment

Ø  Communicate clinical changes to Clinical team

Ø  Provide ongoing case management and 1:1 education

Ø  Transport Clients to Doctor and other appointments

 

To be a success, you must have:

 

Ø  One (1) year of psychiatric nursing required

Ø  RN or LVN/LPT License (California)

Ø  CPR certification

Ø  Caring & passionate

Ø  Experience working in community based programs is highly preferred

 

 

To qualify, you must have a passion for caring for consumers of mental health, strong clinical reasoning processes, proven leadership abilities and demonstrable commitment to recovery. In addition, you must be willing to transport Clients in your car.

 

Applicant must be fingerprinted and receive clearance from Criminal Justice Department.

 

 Bilingual is a big plus!
