Visiting Nurse Service & Hospice of Suffolk, Inc. is looking for dynamic, motivated Physical Therapists interested in working with progressive homecare agency. The physical therapist provides rehabilitation services to patients in their places of residence. Services are provided in accordance with a plan of care to promote positive therapeutic outcomes.
Rehabilitation therapy is provided by a physical therapist to increase the patient’s function and independence and includes:
a. Patient care activities:
1. Initial and ongoing evaluation to determine level of function,
2. problem identification,
3. goal setting,
4. care planning in consultation with the physician to achieve goals,
5. treatment provision,
6. interim evaluation,
7. revision of care plan as indicated,
8. consultation with team members,
9. patient/family education,
10. provision of equipment required to increase the patient’s function,
11. discharge planning and evaluation;
b. Documentation including:
1. timely recording of all evaluation data, treatments, and patient’s response to therapy interventions,
2. recording outcome of conferences, and
3. recording quality assurance data;
c. Education and consultation:
1. with patients and families, and
2. with agency;
d. Consultation with agency staff;
e. Participating in inservice programs
The physical therapist provides therapy in accordance with currently approved methods and standards of practice. The American Physical Therapy Association defines the provision of physical therapy as “the examination, treatment, and instruction of persons in order to detect, assess, prevent, correct, alleviate, and limit physical disability, bodily malfunction, and pain from injury, disease, and any other bodily and mental conditions. The practice of physical therapy includes the administration, interpretation, and evaluation of tests and measurements of bodily functions and structures and the planning, administration, evaluation, and modification of treatment and instruction, including the use of physical measures, activities, and devices for preventive and therapeutic purposes.
The physical therapist is responsible for maintaining communication with the primary nurse and care manager, specifically after the initial evaluation, at least every month, and upon discharge. It is also required that the physical therapist communicates with the nurse, care manager and/or clinical manager at any time when a change in the patient’s status/condition warrants further communication.
The physical therapist is responsible for adherence to agency policies, infection control policies, and for timely submission of charting.
QUALIFICATIONS/PROFESSIONAL REQUIREMENTS:
1. Baccalaureate degree from an accredited college or university with major specialization in physical therapy or baccalaureate degree with physical therapy certificate.
2. New York State license and current registration as a physical therapist.
3. Previous experience as a physical therapist in home care, in a hospital, in a nursing home, or other setting.
