 PHYSICAL THERAPIST AND PHYSICAL THERAPY ASSISTANT 

Southgate Health Care Center,  Inc is seeking a Physical Therapist and Physical Therapy Assistant to join our rehabilitation team. The candidates must be NJ Licensed PT or NJ Certified PTA. Prior experience in sub-acute experience is preferred. Knowledge of Medicare and RAI process preferred. Rotating weekends are required.

Southgate Health Care Center has become the area leader for long-term and transitional living. Our 152 bed nursing facility, offers a refreshing alternative to the institutional hospital setting. We offer a comprehensive range of medical, social and personal services to meet our residents’ needs. The combination of 118 long-term beds and 34 sub-acute beds, in our Transitional Living Center (TLC), allows for a large spectrum of rehabilitation services. Rehabilitation services are offered six days per week in our spacious, recently renovated gym. An in-house run rehabilitation program, four full-time rehab/restorative aides and excellent support from administration allows the focus to be on quality patient care with positive outcomes.
