Based on problems determined, the physical therapist establishes and carries out a plan of care to include patient/family instruction, discharge planning, documentation on the medical record, and communication with physicians and other medical staff. This position helps patients achieve maximum level of physical functioning. Acts as clinical instructor for students and actively participates in department functions. REQUIREMENTS:EDUCATION and/or EXPERIENCEBS in Physical Therapy; Licensed in the state of Ohio; CPR certification. SPECIALIZED KNOWLEDGEBasic computer skills. KIND and LENGTH of EXPERIENCEEntry-level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical experience.
