Job Description: Join a dynamic, talented group of Phsical Therapist! The Mount Sinai Sports Therapy Center is seeking creative, hard working and caring Physical Therapists interested in working in an environment committed to providing the highest quality of patient care. Under the supervision of the Clinical Therapy Manager, the physical therapist evaluates and treats all patients referred by a licensed physician, and assumes administrative duties as assigned. Primary Duties and Responsibilities 1. Performs initial evaluations, establishing appropriate goals, plans and implements treatment programs, performs timely re-evaluations and modifies goals and treatment programs accordingly. Coordinates with other disciplines when indicated. 2. Assumes responsibility for all documentation requirements in a timely manner. i.e, initial evaluations, progress notes, monthly summaries, discharge summaries. 3. Provides training to families or care partners and medical personnel as indicated. 4. Recommends appropriate durable medical equipment in coordination with appropriate team members. Writes letters of medical necessity when indicated. 5. Participates in discharge planning, makes recommendations for continued physical therapy and provides home exercise programs when appropriate. 6. Attends all appropriate conferences, rounds, meetings. 7. Considered as a primary resource person for coverage and therefore expect to carry an increased caseload at times. 8. Attends clinics as assigned. 9. Maintains attendance records, patient charges and statistics. 10. Attends all departmental meetings, inservices and Grand Rounds. 11. Participates in weekly PT inservice programs. 12. Participates in the supervision of Rehab. aides, volunteers and students. 13. Provides inservices where appropriate to PT staff and other disciplines.Job Qualifications: Minimum Required Education Bachelor's Preferred Education Master's Minimum Required Experience 0-1 years Preferred Experience None 1-2
