Patient Care Coordinator is responsible for scheduling, monitoring, and assisting patients with treatment and compliance.
They are also responsible for:
Monitor patient attendance in physical therapy program. Contacts patients who have missed more than one visit per week to inquire about success of treatment and encourage regular attendance. Communicates feedback from patients to case manager for follow-up. Communicates to patients the value of regularly attending treatment programs.
Qualifications:
High School diploma/GED and 2 years of experience in this field
(or a combination of higher education plus experience)
Good written skills
Excellent verbal skills
Phone experience
