Position Description:

Responsible for palliative care management for patients and their families. Work collaboratively with fellow members of the pediatric advanced care team, and other healthcare providers in formulating plans for patient/family. Ability to make independent clinical assessments related to end of life care. Able to guide and develop program planning related to end of life care

Qualifications:

License to practice professional nursing in the Commonwealth of PA.Licensed as a Certified Registered Nurse Practitioner (CRNP) in the Commonwealth of PA.Eligibility for or possession of a national certification as a Nurse Practitioner.Ability to work independently.Ability to work collaboratively through consultation with other health care team providers.
