PROGRAM MANAGER
Program Manager, Sustainable Systems MaineHealth's Partnership for Healthy Aging
Full-time, grant-funded position that will lead the implementation of the grant goals for the Sustainable Systems for Evidence-Based Programs awarded by Atlantic Philanthropies and the National Council on Aging to Maine's Office of Elder Services. This position, co-located at MaineHealth's Partnership for Healthy Aging office in Portland and Maine's Office of Elder Services in Augusta; will plan, coordinate, implement, and evaluate Maine's efforts throughout the state to expand access and significantly enhance sustainability of the Chronic Disease Self-Management Program (CDSMP) for older adults, and coordinate continuous quality improvement activities.
Applicants for this position should possess a minimum of a Master's Degree in nursing, gerontology, public policy, or related field; Experience in development of public policy; grant writing and grants management; excellent oral and written communication skills; including computer skills.
