We are an academic center with medical and pharmacy residents as well as the training of pharmacy students throughout the year. JOIN US NOW as we are seeking a dynamic Full Time Clinical Pharmacist who will be working in our Cardiothoracic/Transplant patient care units. The Cardiothoracic/ Transplant Clinical Pharmacist provides comprehensive pharmaceutical care to solid organ transplant patients through integrated inpatient drug distribution, clinical services, research and teaching. RESPONSIBILITIES: The major job responsibilities of the position are: pharmaceutical care to transplant patients, including drug distribution, patient care rounds, clinical intervention, investigational drug studies, research, teaching and pharmacy representation on Medical Center committees. The following activities are components of the position to assist in meeting the major job responsibilities: - Provide comprehensive pharmaceutical care to transplant patients including: Inpatient care rounds Monitor drug therapy Medication order entry Provide pharmacokinetic consultations Reconcile home medications for admitted patients Facilitate discharge planning and smooth transition to alternate site of care Follow patients listed but not yet transplanted to optimize pharmacologic therapy with regards to co-morbid conditions - Promote and demonstrate rational, cost-effective drug therapy. - Promote education of listed patients with regards to co-morbid conditions and transplant pharmacotherapy - Facilitate investigational drug studies. Coordinate the pharmaceutical care of ongoing research in transplantation Provide in-services for transplantation related investigational studies to the nurses and pharmacy staff - Participate in the teaching programs of the hospital as they relate to solid organ transplantation, including Pharm. D. clerkship students and Pharmacy practice residents. - Facilitate the personnel development in solid organ transplantation for pharmacists. - Provide presentations, publication, and other informative activities on transplantation and other drug-related topics. - Participate on Medical Center committees related to organ transplantation. REQUIREMENTSEducational Requirements: B.S. Pharmacy or Pharm. D. Experience Requirements: One year hospital experience preferred License/Registration/Certification: AZ Pharmacy License Preferred Experience: Experience in Transplantation, Infectious Disease and/or Critical Care preferred. Residency Experience preferred.
