PERINATAL OUTREACH WORKER Neighborhood Health Association is seeking a (F/T) individual who will perform home visits to assist pregnant women and their babies. Worker will provide prenatal education and social services referrals. Requires H.S. Diploma/GED, valid driver's license and reliable transportation.
