[ As part of the Case Management Team, the Outcomes Manager is responsible for the assessment, planning and initiation of ongoing patient and family needs for their designated caseload. Provides significant communication to patient, patient family and other members of the health care team. Graduate of an accredited school with a current NJ license in one of the following disciplines: RN, BSW/MSW or RD. 5years recent clinical experience in an acute care setting is required.
