We are currently engaged in a direct hire/full time search for AWAC, LLC, based in Augusta, Georgia and a wholly owned subsidiary of inVentiv Health.  We are seeking highly motivated, results-oriented professionals for the role of Case Manager in Augusta, GA.  Three (3) positions are available.

AWAC, LLC is a unique medical consulting and cost containment firm working for ERISA self-funded health plans, employer groups, third party administrators, Managing General Underwriters and carriers across the country. AWAC's focus is improving the quality of care while reducing costs for their clients and insureds.  www.awac.md

Responsibilities:

Case Managers facilitate clinical solutions and coordinate cost management opportunities for members with short and long term medical needs. Following national standards and best practices, the key responsibilities include:

 Assess patient / member situation and develop a comprehensive, action-oriented and time specific medical intervention plan to address the needs and requirements of all parties.Implement, coordinate and monitor the plan to assure successful outcomes including clinical, financial, quality of life and client satisfaction.Create periodic Case Management reports.Functions as an effective liaison between patient, third party payer, reinsurance carrier and/or other customers, while implementing targeted interventions.Functions as a member of a multi-disciplinary medical team, including social workers, dieticians, pharmacologist and physician consultants.

Requirements: Education:

 

Required:

Active RN Licensure

Preferred:

BS in Nursing

 

 Certifications: 

Preferred:

Additional Professional Certification in a Clinical Specialty

 

 Experience:

 

Required:

Minimum 3 to 5 years Case Management.

Preferred:

·  Certified Case Manager, Certified Nephrology Nurse.

·  Experience in the following therapeutic areas: Oncology, Renal, Critical Care or Chronic Illness. 

 Skills, Knowledge & Abilities:

Required:

 

·  Excellent verbal and written communication skills, as well as attention to detail.

·  Proficient in Microsoft Word.

Preferred:

Broad clinical acumen
