Nursing Clinical Specialty Coordinator-Adv Lung Disease Program

Job Description: The Clinical Coordinator is a Registered Professional Nurse with broad knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations. The Advanced Lung Disease Nurse will work alongside the Medical Director of the ALD program and ALD faculty to support the outpatient practice, research efforts and support group of ALD.Responsibilities include1. Screening and triaging patients in an outpt setting2.Evaluating patients with MD3. Contacting referring MDs to obtain all data necessary for initial consult and maintain ongoing relationship to provide feedback4. Coordinate the complex management of pts in the ALD program5. Independent review of lab data, monitoring of pt compliance with prescribed regimens6. Telephone contact with patients to answer questions, provide results of tests and address urgent matters7. Call in prescriptions under MD direction8. Coordinate and facilitate required tests9. Maintain accurate and updated records10. Enter data into database11.Establish contact with pts admitted from office for continuity of care12. Develop support group for pts and educational materials for programJob Qualifications: 1Ability to read, write, understand and speak English in a clear and concise manner.2.Visual and aural acuity to assess patient status.3.Communication and interpersonal skills to work effectively with a wide variety of individuals.4.Ability to use basic data entry and retrieval systems.5.Ability to organize information and teach others.6.Analytic ability to collect and organize information, identify problems and trends, and formulate and implement corrective plans or programs.7.Ability to coordinate, prioritized, and allocate resources.8.Oriented to consumer satisfaction.9.Physical stamina, dexterity and mental health necessary to coordinate a broad variety of services and programs for designated patient population on a daily basis.10.Demonstrate the following minimum requirements:a.Licensure:New York State License with current registration. b.Education:Baccalaureate Degree in Nursing is required. Master's in Nursing and certification in specialty area are preferred. c.ExperienceProgressive job related experience specific to area of responsibility.
