Nurse Practitioner: Outpatient Diabetes Center

Advocate South Suburban Hospital is currently seeking a Nurse Practitioner for their Outpatient Diabetes Treatment Center. The Nurse Practitioner/Advanced Practice Nurse manages all aspects of diabetes care for South Suburban/Trinity Outpatient Diabetes Treatment Centers. This position will practice out of two sites: South Suburban and Trinity Hospital outpatient centers. This position is also responsible for reviewing and updating the diabetes center clinical management protocols, reviewing cases with the diabetes treatment center Medical Directors, and other center staff as needed. This position also serves as a diabetes treatment resource for physicians and other disciplines across South Suburban and Trinity Hospitals. The position will also assist in fostering growth of the outpatient diabetes treatment centers and participating in diabetes treatment center operational meetings as needed. The position will also fulfill other duties as assigned by the Director of Cardiovascular Services for South Suburban Hospital. The position will be part-time with part-time benefits with the potential increase to full-time employment and benefits based upon clinic patient volume. â€¢ Masters Degree in Nursing • Minimum 5 years nursing experience • Minimum 3 years experience in diabetes management • Continuing education as appropriate for licensure/credentialing • Nurse Practitioner or Advanced Practice Nurse licensed in Illinois • Current RN license in the State of Illinois • National certification as Certified Diabetes Educator (CDE) or eligible to sit for examination within 2 years of hire required Advocate South Suburban Hospital is a 291-bed facility, providing Chicago’s south suburbs with a wide range of comprehensive inpatient, outpatient, diagnostic and ambulatory medical services using state of the art technology.  The Family Birth Center, which provides a Level II intensive care nursery, offers 16 single room maternity suites, a family centered concept in which a woman goes through the birthing process all in the same private suite.  South Suburban Hospital has a Skilled Nursing Center, a 46-bed facility within their site, that provides a sub-acute “bridgeâ€� for patients that are being released from acute care, but are not quite well enough to go home or to a custodial nursing facility.  Expanded in 2004, South Suburban’s Emergency Department is state of the art, providing privacy to patients and their families.  South Suburban Hospital is located in Hazel Crest, IL.   Interested candidates please email caitlin.canavan@advocatehealth.com , or visit our web site at: http://careers.advocatehealth.com/.
