Bring your talents to our team, and apply today.   Primary Responsibilities:Assessing geriatric cases, ordering lab results and prescribing medication Managing primary and episodic care for the Evercare enrollees Establish and maintain effective relationships with Evercare enrollees, key decision makers, facilities and collaborating physicians Performing physical examinations and working with patients’ primary care physicians Interfacing with families at regularly scheduled intervals Interfacing with the nursing staff, doing clinical in-services, and collaborating with unit managers Documentation in an Electronic Medical Record   Qualifications: Requirements: Licensed NP in New York, certified in geriatrics, adult care or family practice Master’s degree is required 2-3 years of Microsoft Office experience (Outlook, Word, and Excel & PowerPoint) Assets: Long term care experience as well as managed care experience is a plus.
